1) Take the cotter pin out of the axle nut
2) Take axle nut off -- use a big breaker bar to get leverage. Spec is close to 100 ft/lbs.
3) You need two pliers/wrenches. Tighten with bith pliers/wrenches on one side -- otherwise the adjusting nut just spins.
4) Lift the rear of the bike up, drop it down on the rear wheel. Hard. Do this a couple of times.
5) Check chain slack.
6) Put axle nut back on to torque spec.
7) Check chain slack. Redo if necessary.
7) Put cotter pin back in.
